SIWES Form
At 300level students are required to undergo Students’ Industrial Work Experience Scheme (SIWES) programme for 6 months. At the end of the programme, each student is required to write, present and defend a report on what he/she learned in the industry.
Seminar Request Form
Postgraduate Students are expected to present proposal defense, progress and final seminar for thier MSc. or Ph.D research as requirement for graduation.
Result Verification Form
Students who are not satisfied with thier result can apply for verification.
Final Year Clearance Form
All graduating students are expected to be cleared from the department, hence the departmental clearance form can be downloaded below.
